Whether you need general coordination or specialist assistance we’re here to help you live the life you choose, your way.

We work alongside you to ensure your voice is heard, your needs are met, and your supports are working together to achieve your goals.

What is Support Coordination?

Support Coordination is about making your NDIS plan work for you. It’s a capacity-building support designed to:

  • Help you understand and use your NDIS plan
  • Connect you with NDIS providers, community groups, health services and other supports
  • Build your confidence and independence to manage your supports now and into the future

There are three levels of Support Coordination that may be included in your NDIS plan

Support Coordination (Level 2)

Our friendly team will work with you and your support network to:

  • Develop a personalised support plan
  • Coordinate services across health, housing, employment and more
  • Advocate for your needs and help you build independence
  • Monitor your progress and adjust plans as needed
  • Support you through plan reviews and changes

Specialist Support Coordination (Level 3)

For people with more complex needs, our experienced team can:

  • Navigate challenges such as mental health, housing instability or hospital discharge
  • Support you in regional or remote areas
  • Liaise with multiple services and systems (e.g., justice or child protection)
  • Reduce barriers and ensure you can access the right supports

  • Create a support plan within the first month that reflects your preferences and priorities
  • Check in regularly to make sure your supports are still working for you
  • Assist with plan reviews if your needs change
  • Communicate clearly and respectfully, making sure you remain in control
